Best scenario would be to be able to simply install the OMGW, then approve it in the console under Pending Management.
Or, have this check built into the actual tool and tell you in plain english what the issue is :)

While trying to install a Gateway server on an un-trusted domain ( Not a Forest Domain) I found the Microsoft.EnterpriseManagement.gatewayApprovalTool.exe would either Hangup and Freeze or it would crash returning an unexpected error.
After Several Hours and lots of digging I did turn up one note buried deep in blog comments. To that end I wanted to re-broadcast this little nugget of gold and spread the knowlege.
IF THE GATEWAY APPROVAL TOOLS WILL NOT RUN CORRECTLY ON THE MANAGMENT SERVER IT COULD BE BECAUSE THE ACCOUNT YOU ARE USING TO RUN THE TOOL DOES NOT HAVE SQL SERVER PERMISSIONS.
The account using to run the gateway approval tool must have the authority to access your Operations Database and your Datawharehouse data base. I hope this information helps you. If it does please give me a vote. This gives me some feedback if posting notes from experience helps. Thanks Lee
