STATE CHANGE EVENT absence stops any alerting
I've being observing this issue lately for numerous times. It's really annoying, when the alert had to be triggered, but it wasn't, and the some custom scripts, agent, gateway, management server, DB - nothing indicates that something went wrong.
The issue:
In case there are 0 state change events for a SCOM object - object won't be able to change it's state at all, and as a result - monitor won't generate an alertSuggested fix:

Allow tasks to be selected and ran as a diagnostic or recovery task
Have a drop down in the diagnostic and/or recovery tasks for a monitor that allows you to select a task that can be executed manually from the console. Also allow for a library of diag/recovery tasks to be created so you don't have to create a new one with each monitor. For example, restarting services. Or pulling in restart application pool from the IIS MP to allow the monitor to auto restart the app pool.

Deep insight of issues with Graphic UI
If there's option to show users to highlight where's the problem lies, it will be helpful to resolve the issue progressively; Meaning we can tell users, here is the issue and so alert generated with graphic interface.
It's possible and you guys may be aware of the same. It's easier and effective approach.1 vote -

Add ability to select an alert and send an email. This functionality would be separate from the subscription model

Search function in HealthExplorer
When are we going to have the ability to run searches in the HealthExplorer? When you are not familiar with all the monitors an agent may have and you open HealthExplorer to look for a monitor, if that monitor is in a healthy state, I would have to open all the "child" monitors to find the monitor that I am interested in. This happens often, especially if the monitor flip-flops and is in a healthy state at the time when HealthExplorer is opened. It is useful when you can check the state change event history

There should be an option to disable all discoveries and/or all rules and monitoring workflows when importing management packs.
This would allow SCOM administrators to gradually enable/leave disabled monitoring and thereby be in complete control of what exactly is monitored in your environment. It should of course be optional.

All rules and monitors should have a trigger count before alerting
A lot of errors that are triggered is because there are some short disturbance (network, latency etc) in the Environment. In order to keep "false" alerts down you should be able to set a trigger Count before an alert is generated. For instance an alert should'nt be generated before a rule/monitored has triggered three times. Default value should be one but all rules/monitors should have the possibility to change.

Real time monitoring for troubleshooting purposes
Sometimes need to get the same monitoring data trata comes from MP but in real time as perfmon gets. A troubleshooting tool or add- in that get the same data in real time for a short time.

Make IsMonitorAlert property available in Alert Views
It's a very useful property but available via Powershell only. Ability to sort\filter an alert view by IsMonitorAlert would be very nice. Also some GUI 'marker' for alerts that will not be autoclosed (generated by manual reset monitors or with the 'auto-close alert' option unselected) is welcomed.

Create a management pack - "do not create a folder" option
When any new management pack is created (for overrides or a new groups) a new folder is always created. As a result UI has tens of empty folders. It would be VERY nice to have and option 'create a new folder' (unchecked by default) in the 'Create MP' wizard.

Have a contextual performance widgets out of the box available for dashboards
We've seen wonderful new Management Packs as the new SQL MP & Exchange 2013 MP.
Using their own Visualization Management Pack to show contextual performance widgets in their dashboards.It would be nice if we had more of this new visualization out of the box available, not only for performance but also in general for state views as Exchange 2013 uses.
Some custom examples are already out there from Wei Out There (and they are so cool :)) :

Improve Dashboard Widgets to have same functionality as standard views
Whether I use a standard view (Alert, State, Perf, etc) or include the same type of view in a dashboard, I would like to see them have all of the same options especially for filtering. For instance you can filter alerts by object class in a standard Alert view, but you cannot in an Alert dashboard widget.

Set Rules to Auto Close
Have the ability to set Rules to autoclose. Sometimes a server reboots and causes multiple failed Rules and I have to manually close them.

Filter alerts for resolution state and timeframe in the default Alerts view in the right side Task pane.
I would suggest a way to filter alerts for resolution state and timeframe in the default Alerts view in the right side Task pane.

Add the warning and critical thresholds to performance views as lines across the charts.

Create a self service style interface for notifications
Create an interface similar to the Distributed Application designer where users (Non SCOM admins) can search/drag and drop SCOM objects that they would like to be notified on.

Ability to Make dashboards public and not require a login without relying on sharepoint.
This would be best paired with the HTML 5 web console request, as to requiring a plugin to see the dashboards can make impromptu presentations a pain.
But without having to define all users into a read only role, I want to be able to flag content as public and not require a login into the web console to view.
Dashboards get created for projects all the time, and need to have good flexibility on who sees the data.
